E-policy*

  • Duly signed letter from policyholder

*Notes

  • E-policy is only applicable for policies issued after 6 March 2017 and customers who opted for e-policy during policy application.
  • No processing fee is required for this policy
  • Duly signed letter from policyholder

For investment-linked plan:

  • Quote letter

For universal life plan:

  • Quote letter (if the policy is on premium holiday); or
  • Request for change form (if the policy is paid up to date)

For traditional life plan:

  • Request for change form
  • Policy loan application form
  • Policy loan agreement form (2 original copies)
  • Proof of age policy owner / assignee certified by Commission of Oath or Allianz Customer Services Officer
  • Proof of age life assured certified by Commission of Oath or Allianz Customer Services Officer
  • Discharge receipt
  • Application for reduced paid-up form
  • Proof of age policy owner / assignee certified by Commission of Oath or Allianz Customer Services Officer
  • Proof of age life assured certified by Commission of Oath or Allianz Customer Services Officer
  • Conversion will only take place on policy anniversary
  • Application for extended term insurance form
  • Proof of age policy owner / assignee certified by Commission of Oath or Allianz Customer Services Officer
  • Proof of age life assured certified by Commission of Oath or Allianz Customer Services Officer
  • Conversion will only take place on policy anniversary
  • Application of surrender form duly completed
  • A copy of policy owner / assignee NRIC
  • A copy of life assured NRIC
  • Original policy contract

Your checklist:

  • Signature of policy owner / assignee
  • Signature of trustee
  • Signature of witness
  • e-Payment section duly completed
  • Customer declaration section duly completed

Our surrender process:

  • Submit complete document to Allianz Life branch
  • Allianz receives document and process surrender
  • Surrender approved (Approved within 4 working days for standard cases)
  • Surrender payout via e-Payment (Payout within 5 working days)
  • Benefit election option form
